1. What kinds of animals are you interested in fostering? (e.g. dogs, cats, other.)

2. How many animals at a time are would you like to foster? (e.g. one, two, three or more.)

3. How long would you be able to foster an animal? (e.g. one night, a few nights, a few weeks, depends on circumstances, etc.)

4. When would you be able to foster animals? (e.g. In the summer, anytime, sporadically, etc.)

5. Have you ever fostered animals before in you home?

Transporting Animals

If you are interested in transporting animals to new homes or to foster homes please answer the questions below:

1. What kinds of animals are you interested in transporting? (e.g. dogs, cats, other.)

2. How many animals at a time are you able to transport? (e.g. one, two, three, or more.)

3. Are there certain days of the week or times of the day that you are able to transport. (e.g. weekends only, evenings only, anytime, sporadically, etc.)

4. How far are you able to transport an animal? (e.g. 30 miles, 60 miles, 100 miles, etc.)
